The HCUP American Hospital Association Linkage Files are hospital-level files designed to supplement the data elements in the SID, SASD, and SEDD databases. These files are free of charge. However, agreement to the following Terms and Conditions is required.

• I have completed the online Data Use Agreement Training.
• I have read, signed, and agreed to the conditions of the Data Use Agreement that corresponds to the HCUP state database that I have access (i.e., SID Data Use Agreement, SASD Data Use Agreement, SEDD Data Use Agreement). These Data Use Agreements have been submitted to AHRQ or its authorized representative.
• I accept that the terms included in the Data Use Agreement(s) also apply to the HCUP AHA Linkage Files.

By checking “ACCEPT” and clicking on the continue button below, I indicate my Agreement to comply with the above-stated requirements with the knowledge that any violation of the AHRQ confidentiality statute is subject to a civil penalty of up to $10,000 under 42 U.S.C. 299c-3(d), and that deliberately making a false statement about this or any matter within the jurisdiction of any department or agency of the Federal Government violates 18 U.S.C. 1001 and is punishable by a fine of up to $10,000 or up to five years in prison.
